Fill out these forms at home before you come for your first visit!

We pride ourselves at Renaissance Chiropractic Life Center, LLC, in our customer service. We strive to make your experience with us an enjoyable and stress free one.

In order to make your first visit with us more efficient, we have included some helpful hints as to prepare you better for your initial visit.

  1. When you call our office have two possible times in mind just incase one is not available. It is also very helpful to have any health insurance information available so that we can have your coverage available on your first visit.
  2. We can be reached at 248 545-8550 Monday thru Friday from 8:30 am until 7:00 pm and on Saturdays from 8:00 am until 12:00 pm. We also have an emergency after hour’s phone number on our answering machine.
  3. On your initial phone call please feel free to ask for directions or any other questions you may have at that time. If one of our well trained staff members cannot answer a question for you then you can request to have the doctor speak to you or to call you.
  4. **This initial phone process should take just a few minutes unless you want to ask some questions.**
  5. Feel free to copy, paste, print & fill out our New Patient Introductory Form from this site and complete it before your first visit. Just click on the link below. (Note: This is not mandatory but would save you some time in the office if you wish.)
  6. Remember to bring your health insurance card, photo identification and or savings coupons, if applicable.
  7. Your actual initial visit to our office should not take longer than one hour.

We are a BCBS, Medicare, PPOM & Aetna Provider. We accept other types of insurances including Beaumont/HBS, United Health, PBS and many more....just give us a call and we`ll be glad to let you know.
